This procedure has carried the highest complication risk in cosmetic surgery. The danger is specific and well documented. If fat is injected into or beneath the gluteal muscle, it can enter one of the large veins that run through that muscle, travel to the lungs, and cause a fat embolism. That is the mechanism behind the deaths reported internationally, and it is why several countries tightened their guidance.
How that risk is managed. International plastic surgery bodies now advise that fat must be placed only in the subcutaneous layer, above the muscle, never into or below it. When that rule is followed, along with the use of blunt cannulas, controlled injection pressure, and careful angling away from the deep tissue, the risk profile changes dramatically.
What to ask any surgeon, anywhere. Ask directly: which plane will the fat be placed in? The answer should be subcutaneous only. If a surgeon is vague, dismissive, or says they inject into the muscle for better results, do not proceed. That single question tells you more than any photograph.
Why price should not be your deciding factor. Searches for cheap BBLs and discount packages lead to exactly the settings where corners get cut on technique, monitoring, and emergency preparedness. This is not a procedure to shop for on price.
Who should perform it. A qualified surgeon, in a properly equipped facility, with anaesthetic support and emergency provision. Not a day clinic, not a salon, and not abroad on a package deal where nobody will be responsible for your follow-up.
A surgeon examines your body proportions, assesses available donor fat by pinching the abdomen, flanks, and thighs, and checks skin quality and existing shape. Your weight history, BMI, medical conditions, medicines, smoking status, and any bleeding or clotting history are recorded. Photographs are taken from set angles. You will be told honestly whether you have enough donor fat, which option suits you, and what volume is realistic. Some people are told to wait, gain weight, or consider implants instead.
Blood tests, physician clearance, and a fitness assessment are arranged. You must stop smoking well in advance. Blood thinners and supplements are reviewed. Clotting risk is assessed specifically, since this is a longer procedure with a period of reduced mobility afterwards. You will be fitted for a compression garment and told exactly what you need at home, including a special cushion.
Under general anaesthesia, liposuction is performed on the donor areas. The harvested fat is processed to remove fluid and non-viable cells. It is then injected in fine layers into the subcutaneous plane of the buttocks only, above the muscle, using blunt cannulas. The volume is distributed across many small passes rather than deposited in bulk. A compression garment is fitted before you wake.

Shape changes immediately but is exaggerated by swelling. Fat reabsorption settles by 3 to 6 months, and that is when the true result is judged.
This is the standard Brazilian buttock augmentation and the option most people mean. It has the advantage of using your own tissue, so there is nothing foreign in the body, and it slims the donor areas at the same time.

That fat is then processed to separate out fluid and damaged cells, leaving viable fat cells. Those are injected in fine layers into the subcutaneous plane of the buttocks, above the muscle, using blunt cannulas and controlled pressure. Layering in small amounts across many passes is what allows the fat to establish its own blood supply.
The shape change is visible immediately, but you will look larger than the final result at first because of swelling. Not all transferred fat survives. Typically somewhere between 60 and 80 percent establishes a blood supply and stays, and the rest is reabsorbed over the first 3 to 6 months. A good surgeon accounts for this by slightly overfilling. The fat that survives behaves like your own fat, so it responds to weight gain and loss.
You Need Enough Donor Fat. This is the most common reason we decline a BBL. If you are very slim, there may simply not be enough fat to harvest for a meaningful result. Some patients are advised to gain a modest amount of weight first, and some are better suited to implants.
Where there is not enough donor fat, or where a defined, predictable volume is wanted, silicone gluteal implants are the alternative.
A medical-grade solid or semi-solid silicone implant is placed through an incision in the natural crease between the buttocks. It is positioned within or above the gluteal muscle depending on the technique chosen. Because the implant is a fixed volume, the result does not depend on fat survival.
A predictable, consistent increase in projection that does not reduce over the following months and does not change with weight loss. The trade-offs are a longer and more uncomfortable recovery, a scar in the natural crease, and the implant-related risks common to any implant, including displacement, capsule formation, and the possibility of needing revision surgery years later.
Combination. Implants and fat transfer are sometimes combined, using implants for projection and fat for shaping the surrounding contour.
Genuine non-surgical options exist, and it is important to be realistic about what they achieve.
The honest summary. If you want a subtle improvement in shape and firmness and want to avoid surgery, non-surgical options are reasonable and we offer them. If you want a significant change in volume and projection, they will not deliver it, and a course of sessions will disappoint you.

Critical Safety Note : Tell your surgeon about any history of blood clots, deep vein thrombosis, pulmonary embolism, or a family clotting disorder, and about every medicine including hormonal contraception and hormone therapy. Declare diabetes, heart or lung conditions, high blood pressure, anaemia, and any bleeding disorder. Smoking must stop. Declare any previous buttock injection of any kind, including anything received outside a hospital or clinic, because unregulated material in the tissue changes everything about the plan and must be identified first.
Breathlessness or chest pain after this procedure is an emergency. Go to a hospital immediately rather than calling for an appointment.
There is a black market in buttock injections and it causes serious, sometimes fatal, harm.
Industrial silicone, hydrogel, PMMA, and unlabelled fillers are injected in unregulated settings, often in large volumes and often by people with no medical training. The consequences include chronic infection, hard lumps, tissue death, migration of the material to other parts of the body, disfigurement that is extremely difficult to correct, and death.
A legitimate buttock augmentation uses one of three things: your own fat, a medical-grade implant placed surgically, or a regulated injectable used in modest volume by a doctor.
If anyone offers you a large-volume injection at a price that seems remarkable, in a hotel room, a residence, or a beauty salon, walk away and report it. If you have already had such an injection and are worried, come in and let a doctor examine you. We would far rather assess it early.
This is the hardest recovery of any procedure on this website, and it is worth knowing before you commit.
People who do not follow the sitting restriction lose a significant portion of the transferred fat. That is not a rule we can bend for you, and it is the main reason results differ between patients who had identical surgery.

You cannot sit directly on your buttocks for 2 to 3 weeks, which is the hardest part. A special cushion is used, and you sleep on your front or side. A compression garment is worn for several weeks, and there is no gym, driving, or heavy lifting until cleared, usually 6 to 8 weeks.
